Mastopexy may help if you are happy with the overall size of your breasts but unhappy with their shape in terms of sagging or drooping. You may have noticed this “ptosis” with time or following breast-feeding, loss of weight or with ageing.
Mastopexy lifts and restores the breast into its position and helps improve your overall appearance. You may be a good candidate for breast lift surgery if you have one or more of the following conditions:
  • Breasts that are pendulous, but of satisfactory size.
  • Breasts that lack substance or firmness.

During the initial consultation you will be asked about your desired breast shape and to point out anything else about your breasts that you would like to see improved. Preparation for Breast Lift Surgery

The goal of your plastic surgeon and the staff is to make your surgical experience as easy and
comfortable for you as possible. Smokers will be asked to stop smoking 2 weeks before surgery. Aspirin and some anti-inflammatory drugs can cause increased bleeding, so you should avoid taking these medications for 2 weeks before surgery.

Breast Lift Operation

It is performed under a general anaesthetic and the operation takes 2-3 hours and requires a hospital stay of 2 days.
There are many techniques for lifting the breast depending on the severity of the ptosis or breast drooping and deflation, and this determines the extent and location of the incisions and resultant scars on the breast. The excess skin is excised and the breast tissue tightened with stitches. In addition, a reduction in the diameter of the areola (the pink/brown part of the nipple complex) is usually required. The nipple is moved to a higher, more youthful position and the wounds are stitched with dissolving sutures, and covered with light dressings.

The resultant scars depend on the exact techniques employed and vary from only a circular scar around the new nipple/areoalar complex (Round Block technique), to additional scars such as a vertical scar passing from the nipple to underneath the breast. In some individuals, it is necessary to have an additional long curved scar passing in the breast fold.
Your surgeon will discuss which technique is best for you. Most women are happy to trade a well planned, well healed scar for a more youthful, full, and uplifted breast.
  • Minor degree of ptosis
    Minor degree of ptosis can be corrected by a "circumareolar mastopexy": a rim of skin is removed from around the nipple areolar and the resulting wound closed by bunching up the skin and leaving a circular scar around the areola (where scars are often well concealed as the quality of skin is in transition anyway). By making the width of the rim of skin excised greater above than below the nipple, the nipple can be repositioned upwards. At the same time the scar can be used to place implants behind the breast allowing a mixture of mastopexy and augmentation.
  • Moderate degree of ptosis
    Moderate degree of ptosis can be corrected by “vertical mastopexy”. Because this involves the reduction in the skin envelope around the breast and repositioning of the nipple at a higher level, scars are necessarily more extensive: the scars pass around the nipple, vertically down to the inframammary fold.
  • Severe degree of ptosis
    Significant degree of ptosis can only be corrected by true mastopexy “inverted-T mastopexy”. Because this involves the reduction in the skin envelope around the breast and repositioning of the nipple at a higher level, scars are necessarily more extensive: the scars pass around the nipple, vertically down to the inframammary fold and horizontally in that fold.

    The procedure can be combined with breast enlargement using an implant if an increase in breast size is also desired, the implant will be placed in a pocket directly under the breast tissue, or deeper, under the muscle of the chest wall.

    After Breast Lift Surgery

    After surgery, you'll be wrapped in an elastic bandage. A small tube may be placed in each breast to drain off blood and fluids for the first day or two.
    You may feel some pain and discomfort for the first couple of days for a week or more.
    The bandages will be removed a day or two after surgery, though you'll continue wearing a firm sports bra for six weeks. For the first two weeks postoperatively, you should not lift anything heavier than a kettle.
    It is normal to feel lethargic and tired for the first 2 weeks following surgery. Depending on your job you can usually return to work for light duties after 2-3 weeks. A gradual increase in light exercise is encouraged after your first wound check following discharge from hospital, but no sport, strenuous exercises or weight lifting should be undertaken until six weeks following surgery.

    All surgery inevitably carries some uncertainty and risk, however they are small and infrequent.. Breast reduction is one of the most common cosmetic procedures performed each year, and it can be done very safely.
    • Bleeding: There is a 2-3% risk of postoperative bleeding which may at worst require return to the operating theatre for drainage.
    • Infection: If infection occurs it will become evident within one week of surgery and may delay the healing process or result in the development of scar tissue. This may require treatment with antibiotics. In the unlikely event of infection, the ultimate result of the surgery may be adversely affected.
    • Scarring: Scars are an unavoidable drawback to the procedure, but typically quite acceptable to most patients. Some patients may have a tendency to form thick or red scars (hypertrophic or keloid scars). These scars can usually be improved by other measures.
    • Symmetry: Following breast lift, sometimes the breasts may not be perfectly symmetrical or the nipple height may vary slightly. If desired, minor adjustments can be made at a later time.
    • Sensation: Sensation of the nipple may be changed after your operation. It can be “lost”, “decreased”, “sore” or it may even be “improved”.
    • Loss of some of the skin of the breast.
    • Loss of the breast fat (Breast fat necrosis).

    As the healing process is gradual, you should expect to wait at least several weeks to get an
    accurate picture of the results of your breast surgery. Incisions will fade over a number of
    months until they become barely visible.
    You should also keep in mind that a breast lift won't keep you firm forever, the effects of gravity, pregnancy, aging, and weight fluctuations will eventually take their toll again. Women who have implants along with their breast lift may find the results last longer.
    Your satisfaction with a breast lift is likely to be greater if you understand the procedure thoroughly and if your expectations are realistic.
